Concrete write-ahead mediated Tool broker.

A binding names exact executable code/configuration/remote target, not just the LLM-visible tool name. The host must validate grant authority before installing the broker. Tool pointers are non-owning: the owning Agent or Core engine must keep each exact instance alive until every in-flight broker call has completed.
