Controls of one provider family, in that family's own SDK types.

make_provider_request applies the nest of the provider's family and ignores the others, so one ProviderControls can serve every node of a graph whose nodes call different families. Unset members are not sent; the SDK checks every set one at prepare. extensions are top-level request members the SDK policy admits for the origin, sent as given.
